Player & Parent Agreement + Liability Waiver

By signing below, the parent/guardian and athlete agree to the following terms and conditions for participation in AM Elite Basketball Club:

. Assumption of Risk

I understand that participation in basketball and AAU activities involves inherent risks, including but not limited to:

• Physical injury (sprains, fractures, concussions, etc.)
• Illness or medical conditions
• Contact with other players, equipment, or surfaces

I knowingly and voluntarily accept all risks associated with participation in AM Elite Basketball Club activities, including practices, games, tournaments, and travel.

. Medical Authorization

In the event of an injury or medical emergency, I authorize AM Elite Basketball Club, its coaches, and staff to:

• Obtain emergency medical treatment for my child
• Contact emergency services if necessary

I understand that I am financially responsible for any medical expenses incurred.

. Release of Liability

I hereby release, waive, and discharge AM Elite Basketball Club, its coaches, staff, volunteers, and affiliates from any and all liability, claims, or demands arising from:

• Injury, illness, or damages sustained during participation
• Travel to and from events
• Use of facilities or equipment

This release applies to the fullest extent permitted by law.

. Player Commitment & Conduct

The athlete agrees to:

• Attend practices, games, and tournaments consistently
• Demonstrate good sportsmanship at all times
• Respect coaches, teammates, officials, and facilities
• Follow team rules, expectations, and coaching decisions

The parent/guardian agrees to:

• Support the athlete’s commitment to the team
• Maintain respectful behavior toward coaches, players, officials, and other parents

AM Elite Basketball Club reserves the right to remove any player for misconduct, lack of effort, or failure to meet team expectations.

. Payment Responsibility

I understand and agree that:

• All team fees, tournament fees, uniforms, and related expenses are my responsibility
• Payments are non-refundable once the season or team participation has begun
• Missed events or voluntary withdrawal do not qualify for refunds

. Travel & Transportation

I acknowledge that:

• Travel may be required for tournaments and events
• I am responsible for my child’s transportation, lodging, and meals unless otherwise specified
• AM Elite Basketball Club is not responsible for supervision outside of official team activities

. Photo & Media Release

I grant permission for AM Elite Basketball Club to use photos or videos of my child for:

• Social media
• Website content
• Promotional and marketing materials

No compensation will be provided for such use.

. Agreement & Acknowledgment

By signing below, I acknowledge that:

• I have read and fully understand this agreement
• I agree to all terms and conditions stated above
• I voluntarily allow my child to participate in AM Elite Basketball Club
